PPI on GE Capital/Santander

I took out two GE Capital store cards in the year 2001 and from what i remember i was mis-sold the PPI on both accounts.
Problem 1 being i dont have any statements from these accounts and nor do I have a copy of the T&C's either.
Probem 2 being where do i contact to get statements and T&C's
Problem 3 Is it too long ago to claim for this?
How best do I start the ball rolling for these claims?
