Requesting full case notes - address to send request/how long to give them?

I'm going to make an official complaint to the CSA and want to request my full case notes first - all 5.5 years' worth.
Does anyone know the address to send this request to? Do I need to pay/enclose a cheque? How long do they have to send them back, as I want to state that I expect to have this information back by x date in the letter?

Contact the data protection unit csa, I've just sent off for mine. It costs £10 and they've said it can take up to 40 days I believe0

You're best off phoning them and asking for the address - I know for a fact that the address in that link is incorrect. I'm sure your mail will be forwarded to the correct address, but who knows what the timescale would be.
