Its still working fine for me on the link in the first post. Sorry though I can't see the T&Cs because you have to print out the form to see it and I haven't got my printer hooked up. The link still works fine so I'm sure the offer is still on.0 -

Don't assume Boots is the cheapest, even at half price for all of them - a quick Google showed the one I'm considering is actually £3 cheaper in John Lewis of all places!0 -

I just wanted to thank the OP and give this offer a quick bump, as it's still working and will be on toothbrushes bought before 31st October 2012.
I've just bought the 1000, joined supersavvyme, downloaded the form and filled it in, using EAN 5013965951732 for the toothpaste (thanks also LANGO1966!) and received a £10 cheque from Proctor & Gamble less than week later.
I found the form glitchy to print off too (ancient printer here though) but they accepted my best effort.
If you're buying via online via Amazon, Boots or wherever, you can just send a printout of the receipt. (Don't forget to go via TCB or Quidco too).
For anyone whose bought a toothbrush since Feb 2012 (I think) or is buying one before the end of Oct, this is a really great deal!0 -
