Hi,
Your daugher should get a HC1 form which can be either requested online or picked up from the doctors or health centres
If she fills it in and sends it off they will then assess how much she needs to pay towards healthcare costs
The HC1 form can be used to claim help with NHS Prescriptions; NHS Dental Treatment; NHS wigs and fabric supports; sight tests, glasses and contact lenses; and travel to hospital for NHS treatment.Weight Loss - 102lb0 -

Do you know if she has to put her home address on it, i.e. OUR home address or the address of the halls where she is staying during term time and would her doctor be the one she had to register with during her first week at Uni0
