Change in Credit History

2 days back through a mortgage broker, under the name "Agnelo Fernandes" I was told Halifax is happy to give 75% LTV (loan to value). Now all my bank accounts / credit card / rent payments, etc are done in the name "Agnelo Fernandes"
I submitted my passport, where it is written given name: Christopher Victor and surname: Fernandes Agnelo, under this Halifax dropped their support to 65% LTV.
I am really concerned now that my credit history is good under the name "Agnelo Fernandes" as i have been paying my bills sincerely under that name but when a credit check is done as per my Passport identity "Christopher Victor Agnelo Fernandes", it obviously does not recognise my credit history.
Can anyone please suggest/guide the best way to resolve this issue? My colleague was talking about getting an affidavit done stating that the 2 names is actually the same person. This is fine but then what do i next with the document? Should i be contacting my bank/credit/rent/ companies stating that the two names are same?
Please throw in your ideas

Why do you conduct your financial transactions under your surname only?
Your broker should have had sight of your ID prior to application which would have meant your full name on the application.
Using an alias will cause issues. I had made my passport in 2000 and it was error on the side of the Passport Office which i should have corrected then stating my first/surname clearly.
I have always used Agnelo Fernandes in my school as well as college/uni etc.
Will doing an affidavit help in this case? Stating that both names are the same person. Will sending that to a broker requesting to carry out checks under the appropriate name, help the situation?0 -
So you knew your passport was incorrect yet chose to continue with it and use the new name for all credit?

I am eligible for UK Permanant Residence in August but need to sort this out the name issue before going ahead in it.
I can change my name in the Passport so that it matches my credit history, will that have any implications though on the credit records?0
