Taking your name off a mortgage

Can anyone help me, I'm looking for the legal term used for removing your name from a mortgage following separation. It's nothing to do with selling the property or buying the other partner out, it's purely to do being officially removed from the mortgage fully. I'm sure it's a Deed of somesort....

Where I am it's a 'Transfer of Title' and you have to get agreement from the mortgage lender they are happy to approve a sole mortgage in the other persons name0
