Bankruptcy Default on my credit report dating back four years?

Hi all,

Ok I was declared bankruupt in June 2008, the only accounts I defaulted on were;

-A Hire purchase through RBS
-Overdraft TSB
-Loan TSB

I was discharged in Feb 09. I applied for a small car loan recently due to moving jobs but fail all credit checks so I decided to check my rating (Which is 965). I have defaults on all three accounts dating March 2012 so I was advised to write a letter to the creditors with a copy of my discharge certificate (I have this) explaining that I should not receive defaults post dating my bankruptcy.

Can anyone confirm that this information is correct? And can anyone please link a letter template as I'm awful with writting formally?
