Debts listed twice on credit report?

Hi - I have checked my credit report and all my debts are showing twice and my total debt outstanding is double than what it really is - can this be right?
I lived in a flat for two years and then moved again 2 years ago. Could it be because I have two addresses? Still doesn't seem right though!!:(

It's not a lloyds debt by any chance is it? Have seen a few instances of this very recently so they may have a glitch in their systems.Total 'Failed Business' Debt £29,043
