Declined mortgage for arrears on a utility bill.

I have recently tried to increase my mortgage on my house and I have been declined for arrears on a utility bill.
I have run a credit report, which showed no arrears and contacted all my utilities, who have confirmed no outstanding debt.
Does anyone know how to find out which company this arrears belongs too and how to clear it?

There are 3 credit reports. experian, equifax and callcredit i think is the third.
Check all 3.

As ACG states, it is likely that you have Experian and it will be showing on Equifax.
If the lender has that information and have shared it then it will be there.
If you can prove that it is not you, you can have the negative entries removed although this is a timely process.
