Pregnant while already on maternity leave

Hi
I am currently on maternity leave, and have just found out I am pregnant again (not planned).
I am currently taking my full 12 month maternity allowance, which means I'll be about 4/5 months pregnant by the time I'm due to go back to work.
From what I understand from the government websites:
providing I have been in continuous employment without a break I would still be entitled to the same maternity benefits.
Also that whilst on maternity leave this is classed as continuous employment.
Am I understanding this correctly?
I have requested a change in hours (from full time to part time), and have been told that I will need to sign a new contract - will this effect my 'continuous employment' rights in terms of maternity pay?
When do I tell my employer I am pregnant again, before or after making the agreement for a change in hours? Before or after I go back to work?

It won't affect your continuous employment, however your second lot of maternity pay will now be on the part time rate (assuming that's above the governments basic).
I believe you have to tell your employer at a certain time (weeks gone), not sure when that is.
