Bit stuck on the S&S ISA transfer form. Help please?

Just a bit stuck on one section. I'm looking to transfer SOME (not all) old money from a previous years cash ISA.
ISA providers name: this will be Halifax then.
Providers post code: my local branch or Halifax Leeds? Does it even matter?
ISA reference to be transferred: excuse me? I don't have a reference.
Approx transfer value: It wont be approx, it'll be exact. I don't want them transferring an extra £xyz saying "oh well it was approx what you wrote down".
Cash ISA sort code: It's great that they're asking for this, but what about the account number? Or would i put the account number in with the reference field?
Also, the fields to write which fund you want to invest in are pretty poor. They're quite small when some funds have silly long names.
