    Maybe worth pointing out that inserting your 'car insurance renewal date' is only:

    Mandatory field if you have car insurance


    So good news if you have not yet won that car!

    You can enter without date.

    I agree this is very oddly worded.

    The source code for this line reads: </span><input type="checkbox" name="opt_out" id="id_opt_out" />

    I'm no expert on html, but I'd say this suggests that the tick box is to opt-out of contact.
