Filling out a B16 (JSA)

Hello there!
In what seems like a retrospectively bad idea, I did a day's one-off freelance work before christmas for some CV expansion, which was all declared to the Jobcentre. This was all well and good, until I realise they had suspended my JSA until I submit a B16 form (I ended up receiving the form 6 weeks after I did the freelance work). Unfortunately, half the form hardly makes any sense to me, especially the bit about balance/profit or accounts etc. Bearing in my mind I have no desire to start a self-employed business and the one-off nature of the work, are there any guidance notes for filling the B16 in?

What page are you on and do not understand?
They have given you that to fill in as they belive you are self employed etc like it says on the first page
Assets are anything you own for the business. Say you were a designer you would have a drawing desk, pens, pencils, paper and whatever. These are all owned by the company and if you are signing on they are saying you should have no assests so it's asking what have you do to sell them

I should declare up front that I've never filled this in or even seen it before but.....
I would tick lots of No boxes (except perhaps Yes to the first questions on pages 2 and 3, and some of the questions on page 4 if you might do more freelance work) and just explain what you did in section 10.0 -

I did some copywriting work, at home, on my personal laptop (this was for one day). Basically, inputting content (Q&A) into Excel; now it seems hardly worth the effort. I have done no more copywriting work since then, and have no intention of doing anymore self-employed work. I am in the process of declaring the income to HMRC and I am trying to show that this is one-off employment rather than me trying to set up a business.
Basically, my JSA advisor said that I should fill out as much of the form as possible as it pertains to my case, but as much of it hardly seems relevant to my case (Business assets, yearly accounts, average hours worked etc.)0 -

Is there a number to call on the form or call the JC helpline and ask them maybe?
From what I saw of the form it really looks like its meant for people with equipment and stuff.0
