I filled out form for this one, and got the subsquent email confirmation that says in order to get the sample I have to send postage stamp for a large letter to their address. So it's not strictly speaking a free sample contrary to their implication, as it requires you to pay postage.

Ive just entered my details, waiting for the email now. Once you submit your details it also gives you a link to request a free sample of a boots product, think it was MD? Worth a go for 2 free samples anyway!
