sc cockaleekie soup recipe?

I've got 3 leeks to use up, and I wanted to make some soup. The kids don't like leek and potato soup (I don't mind it, but couldn't eat a whole family's worth!), but I've some chicken portions in the freezer so I thought I'd make some cockaleekie tomorrow and see if they like that. But I want to do it in the slow cooker rather than on the hob as I've stuff to get done tomorrow. Anyone got a good recipe to share? I'm not quite familiar enough with converting recipes to use the ones I already have.
