Self assessment – Limited Company

My apologies if this has been asked before but a search has not brought it up
In April 2010 I set up a limited company to trade as a consultant, I have invoiced clients via this company and have had company accounts prepared. The Corporation tax for the company has also been paid (Approx £5,500)
During 2010/2011 I lived off my redundancy payment and savings and did not pay myself any salary from the company. Being a widower and still with a son in school I also received a pension of approximately £5,000.(which I no longer receive)
I have many questions about my Self Assessment but perhaps I should start with the basic ones.
Incidentally when I was in full time employment I completed my own self assessment for the last 10 years but as my circumstances have changed I now could do with some advise.
A.In Section 3 Taylor your return I have said Yes to being an employee, director, etc and then provided the name of the employer - my limited company. I have said No to being self-employed and No to being in a partnership. Is this correct?
B.In section 4 Fill in your return I need to include an Employer’s PAYE reference but as I have not paid myself anything I do not have one and I do not have a P60 or P45 (Perhaps I should set that all up even though the company has not paid any salary or NI contributions etc. yet)
C.Should I have said No to being an employee , director,etc as the Company has not paid me any salary.
I will be paying myself a dividend from the Limited Company so should all my tax liabilities be included in the dividends section as well as the Widower Pension as mentioned above.

    If you don't have an employers reference number, the workaround is just to enter your personal UTR (unique tax reference).

    You enter the amount of dividends actually paid/voted in each tax year. If you didn't vote nor draw any dividends in 10/11 tax year, then nothing to enter on your SA return. It doesn't matter when the profits were earned, it's the dividend date that matters.

    Yes, you do have to declare your widows pension as received in that tax year.

    Many Thanks for the response. The intention is to take my dividends as a lump sum for that year, will check that they were 'voted' but was advised by my accountant (for the Ltd Co not my self assess) that it was money I could draw down from the Company.

    So does that mean I put No in to being an Employee and then complete the Dividend section, sorry but it's not convenient to access the HMRC web site for a while so it may be self explanatory when i get back on there.
