HELP!!!!! job offer maybe withdrawn....

Hiya all am a newbie here, and am having a few issue with a job offer, and and references from a pervious employer....
am a qualified nurse, and i worked as a staff nurse for 2 year in an a nhs hospital, i left to study a post graduate diploma in health visiting which i finished in september, I have since being offered a conditional job as a health visitor, I did the interview all checks crb, occp health, reference from uni to confirm health visiting course and a work related reference to confirm my performance have all come back positively...
However I received a call from hr asking me to provide details of my manger whilst working as a staff nurse which i have, but this manger, I have since been informed by hr has so far refused to give me a reference, now I know I was a good employee I had no issues whilist working there, I can't think of any thing in my performance to warrant her refusal... the only thing thing i can think of is at the time of leaving the job. I was off sick in the last week of my notice period... I spoke to her at the time and she seemed to understand... also I did not have a record of being sick constantly, i had 1x period before that episode... so am not sure thats the issue... i have tried contacting her and she seems to be avoiding me....i have provided the name of another manger in the same ward but am not sure if she will do the same thing...
the thing is now, I am really worried I will have the job offer with drawn, i really need this job and I feel I have worked really hard in the last year to pass my course and i feel this is unfairly effecting my chances of ever getting another job.... hr have said to leave the issue them and they will get back to me....am thinking of contacting my potential new line manger and explaining the situation...
what do you think i should do???

It might be that 'refused' is actually 'been unable to' as she's on holiday, or something? Maybe suggest to HR that they contact the HR of your former employer if they're having problems getting hold of the old manager.0
