Default on credit score- Help!

Hello i wonder if someone can help me with some advice.
Last October i signed up for a new contract phone with Orange i set up my direct debit and i thought everything was OK until my second bill came through the door and it was twice the amount of the agreed price of the contract, so i called Orange to ask why this was. I was told it was because Orange and my bank hadn't set up the direct debit in time for the first months payment so it had rolled over to the following month- which was fine by me. The orange guy asked if i wanted to pay over the phone and then he would cancel the DD for that month and it would return to normal the following month. I agreed to pay the money over the phone and i left it at that.
The following week i received a letter from my bank saying that a direct debit had effectivly bounced because i didn't have enough money in my account (because i'd already paid the bill!!) I was charged £25 for this of which i fought Orange to reimburse me for which they did in a sense (it was put on my Orange account). But yesterday i went to my bank for a loan and i was rejected and told it was due to a default on my account!!! I'm extremely worried as i wasn't warned this was happening and I'm getting married next year and we'll need to get a mortgage but i fear this will be impossible to do with this on my credit score.
How do i get rid of this default as it wasn't my fault? and who do i need to contact to remove it? Orange? or my Bank?
Any advice welcome.

Get a copy of your credit report, as you currently have no proof that it is the Orange account which is showing as a default.
Find out who the default is with, then you can get in touch with them to have it removed, if the circumstances are in your favour.

once they have removed the default how long until it disappears from my experian report?
