Vaccinations for work?

So I work for a hospital and all I needed to do was fill in an OH form, have a blood test, say I'd had chicken pox and show my BCG scar.

Now to join an agency I need to show evidence of:

Varicella - signed declaration or serology report
TB - Prove of BCG scar signed by GP or prove of positve test result
MMR - Serology report or individual lab reports
Hep B - report showing titre level greater than 100ul/ml

I went to my doctors and they printed out a list of the vaccinations I'd have.

I have now booked an appointment to see my GP in the hope that they can supply me with reports for MMR & Hep B.

The list I have doesn't include Hep B but I'm sure I've had it, if I haven't can I get it done due to needing for work? Would they charge me? will my GP supply me with reports?
