Help with ridding me of my creditor demons

Hi, I need a lot of advice and help with this. My debts although seemingly huge in significance to myself seem to pale against others. Basically. I owe;
Capital One - £1900 (minimum payment of £88 high interest)
Overdraft - £1800 (never out of it and now ignored as if it's my balance)
Very - £1000 (a lot of buy now pay laters - due to start July 2012)
Dell Laptop - £220 (£20.01 per month)
My monthly expenses are quite high and although I may have minimal funds after paying all my bills and never missing a payment off my debts I can't afford to pay more than they ask and the interest is crippling. I also can't seem to stop spending on them. I paid my credit card down to £100 and then they upped the limit I could spend from £800 to £1900 (I had a holiday coming up and expensive car repairs so maxed it out). I know I don't need the crap I buy off Very and i could probably have acquired money else where rather than the Credit Card. I understand I need to snow ball my debts and pay off the CC first. Is there any chance I can get Very and Capital One to freeze my account so I can't spend on them? Also would they let me either reduce the interest or freeze the interest on them? Can someone provide me with any sort of template I could use to send to these companies requesting they don't let me buy/spend with them?
Your help is really appreciated. I'm so worried about my situation I've considered transferring to a 0% card. I checked my credit status and it is at 730/1000 but I have been refused a transfer card in the past and do not want to apply and damage my credit score anymore than I have the last 6 or 7 years, as it's taken me so long to get to 730.
