Remortgaging and then moving

Just wondered if anyone could give any advice in regards to my current predicament.
I have a Northern Rock together mortgage which is now at 7.4% (had for 6yrs). I would like to remortgage to get a better rate (flat now worth about 2.5 times the mortgaged amount ). Problem being that I am looking at selling up and moving back to the country hopefully within the next six to eight months.
So is it worth remortgaging to get a better rate now or just putting up with paying quite high for the time left before moving. Are there charges to remortgage? and then when I move I’d have some extra things to deal with or can you move a mortgage to another property at a different price and add another person on? and if so is that going to be costly? Also what do you remortgage the original amount borrowed or a higher amount or ?
Have only had to deal with mortgages the once so far i.e. when I bought the flat in December 2000 so total mortgage idiot here.
Thanks in advance for any advice.
