Payday loan debt

In september I took out a payday loan, and it rolled over, then I had to take another one out and I am now in the situation where I have 5 to pay.
I have made an offer of payment to the companies, and 1 of them has sent me an Income & expenditure form and wants a copy of my bank statements.
I am happy to fill in the form, except for the fact that it wants to know my wife's income.
Do I have to put her information in as the debt is in my name? Or do I just work out how much we pay pro rate from our wages and put my income in modified to take that into account?
Do I have to send them the statement?
I keep on asking the company for payment information so I can set up a standing order but they say I have to give them the information first.
Some advice would be great.
