I also use the Thermos wide mouthed/neck flasks for my son (I did test them at home first to make sure the food was still at a good temp) he will have pasta & HM sauce, minestrone soup (with some pasta stars in) a thick soup usually veg based/tomato (HM)
If he helps me make sausage rolls, then he will have these or dried cold toast with a pot of pate, or HM spread

We also made a chart which has
Carb Protein Fruit Veg Sweet then a list (which son choose) of say wraps, cold toast (for the pate or spread), crackers, rice cake etc... Then turkey, ham, cheese etc... The fruit he will eat (nothing cut up that will go brown!!!) then Veg Cucumber, Carrot sticks, Celery etc...
He then chooses one from each list when we make packed lunch, and has gradually go to the point where he usually eats most of it
I must add I got sick of throwing food away and having a tired, irritable, hungry child come home each day
