has anyone else been required to fill in a form to request their p45?

Your employer is required by law to give you a P45 when you leave, so there should be no reason to have to fill in a form to request it.
But the mere act of asking you to fill a form in is not in itself unlawful, but if they refuse to give you the P45 having requested it, form or no form, that is.0 -
Who's asked you to do it? What is "the form"?
They must initially provide you with one as reasonably soon after you've left as possible though there is no mention of a strict legal time limit on HMRCs site. http://www.hmrc.gov.uk/employers/p45-employee-leaves.htm0 -

my ex employer and the form - i haven't a clue as i haven't seen it yet - i was under the impression employers sent p45s out automatically when you left and i wondered if this 'form' was a new thing employers did0
