Lloyds TSB & Santander debt recovery for credit card

I am confused!
In July I spoke to cccs after Lloyds wouldn't accept a payment offer as I have no money, CCCS helped me write to all my debts offering payment of £1 a month.
All of them accepted; however today I received a letter from SCM solicitors on behalf of Lloyds regarding the debt and asking for full payment or face legal proceedings they also said I could contact Lloyds and make a payment offer.
So I called Lloyds and was told to ignore the letter as there is already an agreement in place.
I'm not sure why they sent this letter if there is already an agreement?
Do I really just ignore this letter?
Santander also accepted payments but have sent similar letters and one clearly said default notice. Is this the same thing? as they said they would leave it 6 months before contacting me again about increasing payments and checking my circumstances.

If you have stopped paying the minimum amounts and set up a debt repayment plan, you will have defaults on the debts. However this does not mean that they have not accepted the repayment plan sorted out through CCCS.Savings target: £25000/£25000

Because they are useless and incompetent?
