    Hi there,
I sympathise - it sounds like you have trapped a nerve in your back. I've done exactly the same thing as this before and know the pain you are in.

If you are ill for more than 5 days, you'll need to provide a Drs sicknote to your employer, but for periods of less than 5 days you need to self-cert. Effectively that means 'tell your boss whats wrong with you'. Some companies require you to fill in a form for this, some are happy with a verbal message and will make a note in your file. Unless your contract says any different, there shouldn't be anything sinister to this that you need to worry about, and if you are in as much pain as I imagine you are, it should be fine for you to complete the form on your return to work. You don't have to struggle in just to fill it in.

I really hope you feel better soon. When I've done this in the past I've visited an Oestopath or Chiropractor who has put things back in the right place very quickly.
You had me at your proper use of "you're".
