Credit report help

My OH's credit report only has one debt on it, which sounds fab, except it's not right. There should be two bank accounts - overdrawn, a DCA chased him for something earlier this year, although he couldn't pass the DPA, he has a phone contract which has a few late payments, shouldn't these all show up?
There have been an awful lot of address over the last 6 years due to what he does, is there a possibility that there would be more than one report, As a lot of the most recent addresses are missing?

Also the debt that is on his report is a DCA that says he had a phone contract but it isn't in his full name, it's his middle name and surname.Then at the linked addresses it says that this DCA gave a linked address in 2010 of somewhere he lived a few years ago.

I'm not sure what to do as it's actually details of one of the bank accounts we needed.
