Transfer State Pension from PO to Bank

Can someone please tell me the procedure to transfer a state pension which is corrently paid into a PO card account into a bank current account.
The person is having difficulties getting to a PO but could hobble to an ATM nearby.
Can a form be collected from a PO or does it have to be applied from the government agency.
BTW the lady already has the bank a/c -Thanks.
