3 year bar makes me mad

Hi Guys & Girls,
Letting a little steam off today, last week a letter from Phoenix saying too late three years are up, so thought lets see what documenatation do they have and they have sent me copies of the recommendation form. In the form I have objectives of having adequate cover and keep mortage down and another section where it meant to have details including term, sum assured etc its does not have the detail written down. If only the time bar
