Mortgage application - is there an issue?

We submitted our mortgage application about 3 weeks ago, and we still haven't had our formal offer.
We have chased the lenders several times, and each time they say theres no problem with our application, but there is always a reason why the offer hasn't been made.
The last reason was that it is with the underwriters, but there is a computer problem and they are waiting for someone to overide something before the offer can be made. This was about a week ago.
Does this sound like excuses / stalling?

You only receive your offer after you have had a survey done on the property - has the surveyor been out to complete their report or are you simply at application stage ?
Are you using a broker ?
