I have looked over documents again and it was actually a mortgage offer from Halifax 26th May to Sept 30th(instead of AIP).
The Valuation was done on the 3rd of May which was requested by Halifax. Paid £315 for valuation
In the credit report listed searches by Halifax as follows

    1) 26/4/11 First Mortgage which time at address was incorrect 1 month

    2) 27/4/11 Unrecorded entry by Halifax mortgages £94,500 term 26months?

    3) 27/5/11 Unrecorded entry by Halifax mortgages £94,500 term 25months?

    4) 27/7/11 Halifax First mortgage incorrect address again 4months?

27/7/11 was when Halifax verbally withdrew/changed original offer from 90%to 85%LTV. Also we have had nothing in writing to say they have changed offer or an explanation?

    Can anyone help please?
