Travel fridge for medecine storage

My relative needs to transport his medecine around the country at constant fridge temperature.

He has been told that the portable cool box type of cooler sold by Halfords etc is not efficient or cold enough.

I have looked online for something small which can be plugged into the car lighter with no success.

Surely diabetics have the same issue? Does anyone have any ideas?

    What is his medicine? Is it something he's going to be taking for a while or just short term? If he really has to travel while he's taking the drug he should ask his doctor if he can be prescribed something else that'll do the same job but in a different formulation, either tablets, aerosol or possibly injection. Even if the exact same drug isn't available in a different formulation there must be a chance there'd be another in the same class.

    Also when in use, it should be kept out of the fridge, it's OK out of the fridge for ~30 days (the exact length of time depends on the type of insulin).

    The medicine is teriparatide. He has to inject it daily for two years as a treatment for osteoporosis.

    I've had a look and there are alternative treatments for osteoporosis so he should ask his doctor if there's something else he can take that is logistically possible. Given many people's lifestyle would make storage at those temperatures impossible, and as it is meant to be used immediately after removal from the fridge, there must be ways round it. A one off probably wouldn't make too much difference (though I'm no expert on full stability profile etc) but as he's got 2 years of constantly having this problem it's unrealistic.
    I did have a mini fridge that plugged into my car cigarette lighter but it didnt hold the cold very long once unplugged and was more hassle than it was worth
