Occupational Health questionnaire and confidentiality of information

I work for Morrisons. Last week we've been given a Night Workers Health Questionnaire, which states that -when filled in- it should be returned to the company's OH department in an envelope provided.
My question is: If the document should be returned to OH dept and envelope has not been provided, then is it legal for personnel dept to demand its return? In the end I believe that any information in the questionnaire are confidential and should be seen only by OH department?

Of course it's legal for them to ask you to return it - if you don't have an envelope provided, ask for one, or if it's Freepost, copy the address from another colleague.
I wouldn't send it to your Personnel dept, but just ask them for another envelope, or for the internal address so you can send it via internal mail.

!!!!!! OP, get a grip, you are worrying about effectively whether it is legal or not for the OH departmment to have provided (or not) an envelope - value about 5p, to maintain confidentiality!!!???

Hey guys its not about an envelope or a stamp for which I will happily pay if Morrisons can't afford.. maybe my wording wasn't clear - my apologies..
OK so say if I am 'not welcomed' disease carrier, then not necessarily want for everyone to know about it? I'm not saying that I am, but its not the matter for personnel to know about?
Or if I'll write 'I've got problems with my back because workload is too big for such a small night crew' then following week I can expect a redundancy letter from the store manager (you know get rid of him just in case..)
Actual reason I am asking is that my line manager asked me to take the questionnaire of one of absent colleagues home with me, so that he could fill in 'supplementary data', then I'd bring it back following night (we live in the same town and drive a distance to work together).
In this case personnel, line manager and myself know what bothers Mr.Smith? And if I were a !!!!!!, then entire store would know about that already? This is why I ask about confidentiality of information.... not the envelope.....0 -

I wouldn't take anything not in an envelope for a colleague. But then I wouldn't expect my boss to give me anything not in an envelope to deliver to a colleague!Signature removed for peace of mind0

You can return these direct yourself if you wish - It just needs to be addressed to OH at Hilmore House, Otherwise put it in an envelope and give it back to Personnel who will send it in the recorded internal mail.:dance:Quidco Payments In 2011 - £724.21 :dance:

I got one of these tonight also altho i work in a different supermarket, you can opt out of filling it in there is a declaration at the top of the form , i myself dont have issue with the questionaire but i do take issue with them asking me to fill out a consent form to keep on file to access my medical reports,0

A bit harsh, it's clearly not about the envelope but into whose hands this will fall. We all know this could be potentially difficult for the OP if it does go astray.!!!!!! The form is to go to the OH department and not for anyone else's eyes therefore, OP, I would either call the HR department and get them to send out an envelope if there is one specifically addressed to the OH department or ask them for the direct address and contact name.0
