Nationwide default on Experian how,why?

I have had an arrangement with KPR on behalf of Nationwide collecting for my overdraft in 2010 which was cancelled after years at two weeks notice for 1700.
I have been paying it off by arrangement with KPR after being advised to change banks by my debt advisor. Nationwide refused for six months to speak with my debt advisor and kept ringing myself and my wife and we said please deal with my debt councillor who had also sent them on two occasions letters of authority at least one of which I believe was registered. I said repeatedly that I wanted an arrangement to pay.
In May 2011 a Jan 2011 default notice was added to my account by Nationwide. This stops me remortgaging or getting a car loan at a reasonable rate that I will need for work. I had even been offered a consolidation loan that would save me 250 a month but due to this addition on my record I am stumped.
I contacted KPR who said it was Nationwide and agreed that this cannot be right as I had not missed any payments on our agreement for two years. This was then amended to a number 6 saying I have missed six payments every month until this month which is blatently untrue and still stays on for six years.
On calling back I was told this all relates to 2010 when for six months I refused to communicate with the bank and I was defaulted as a bad debt. Again blatently untrue.
Even if I accepted that premis I am sure they cannot start putting this on a year later?
I spoke to experian this morning who say this is not good practice and agree with me and have advised I request a DATA SUBJECT ACCESS and write to the FSA and complain. They are concerned that this is the tip of an iceburg as I was told that the bank had written the debt to bad debt which suggest an insurance claim perhaps when in fact I was desperately trying to sort the debt out?
Strangely my account now says that I owe nothing but am still paying the debt.
I was advised to ask if this is happening to anyone else?
