Form 6.28 questions

Hi again,

On the form 6.28 under list of unsecured creditors, if I don't know the Account reference or agreement number, can I just leave this blank or will it be difficult for the OR to locate the account?

Its mainly for things like, Wonga.

I also can't find the account number or address for a HSBC credit card that is currently secured against the house we voluntarily repossessed. This is on my credit file but I have no way of finding this info out, The last I heard it was with CL Finance (i think)

Ok now i'm at section 4. For the list of unsecured creditors, do I put the name of the actual acompany the debt was with to start or the debt collector it is now with?
I.E - BT PLC - now with Credit Security Limited

I just put down who the debt was with originally.

Thank you. I wish someone could fill the form in for me, I hate writing in capital letters, my writings to big lol.

I'm thinking of attaching a copy of the last letter from all my unsecured creditors, would this be acceptable or not? Just to make it easier as my writing isn't the best to read all jumbled up in those tiny boxes
