Default on my credit score

I have a default on my credit score, I have just checked it .. I was expecting more.
It was for 3g UK.. so the three mobile network, I do have a mobile phone contract with them but that's the only one I have ever had. This shows on another entry as satisified.
How do I go about getting this investigated and possibly removed?

So which entry is correct? Did you default on your payments? Do both entries have the same start date on them? Do you live with someone with the same surname and the satisfied entry could actually have been their agreement? Sibling/parent etc?
Which CRA did you do the search with? It could be that if you did a search with one of the other agencies (Experian, Equifax, CallCredit) the other defaults you were expecting will be there as not all creditors report to all 3 agencies...0 -

The ongoing one is correct, the start date etc match up.
The enteries have different start dates, the default is for only a period of 12 months.. and is set as satisfied. My phone contract has always been 18 months.
I am the only one at the address with this surname, my mother has a different surname as she's remarried.
I've checked the others, this is my only default. I just assumed because of latepayments etc that i would have more but everything else matches up.0
