Credit report - Notice of dissociation

When I was young and naive (before finding this site) I got a joint account with my flatmates when I moved out for the first time. It worked out well at the time, however I now realise it has left me financially associated with 3 other people on my callcredit credit report.

I've downloaded the notice of dissociation form however I don't have the details of the current addresses for my ex-flatmates and I was wondering if they will accept the form with this omitted or if this is essential?

    Try submitting it without (just write unknown or last known address was such and such)- hopefully it should still go through.
