the more notice you give your employer the more chance they have to consider your request, if refused you will have to wait another year to submit another request.Be Alert..........Britain needs lerts.0

Could I suggest that you will also strengthen your case by outlining how the job gets done while you are working part-time not full-time? You don't have the right to come back part-time, and since your employer can make a business case for not allowing it you would be well advised to make a case for allowing it.
It might even be worth speaking to the other lady currently on m/l and seeing if teaming up and offering yourselves as a job share would work.Signature removed for peace of mind0 -

There is lots of information here on the process. Your employer has to seriously consider your request and if what your asking for cannot be accommodated, they should explore alternatives with you before declining on business grounds. You should be able to get the forms from your HR department which would list the business grounds on which they can refuse so that you have a heads up. When stating your case, make sure that you have explored how your request is likely to impact on your team and how you may mitigate it. In the unfortunate event that your request is refused, you can appeal and your HR department should also provide your with details of the appeal process. It is always advisable to speak to your line manager ahead of making the request so that you can discuss any likely issues and how they can be addressed. It's just courtesy. However, if you don't feel comfortable doing this, they still have to meet with you within 28 days of you making your request to discuss it. In my company (large accountancy consultancy firm) it can take up to 14 weeks for requests to be considered and sorted so always good to get in as early as possible.0
