I finally got round to sorting out the PC in line with your excellent guidance. Commit charge is now 358420 - PC is noticeably quicker although when using the internet I have to run CCleaner every hour or so - any other help you can offer greatly appreciated.
I re-ran the hijackthis log as just below. Also I appeared to have had 12 infected files when I ran and purged them with Malwarebytes, that log further below:

I followed Closed's guide too and my pc has been working great ever since.
My only problem is that same as yours an absolute deluge of cookies and they're mainly from MSN (files and Jpeg images) they all have a prefix of estb or etsb before them. Doh! I went and did the CCleaner and they haven't come back now!
I have just gone through the guide again though so maybe that's why the problem seems to have rectified itself. If not, I'll be back here later!
Good luck Podgy, Closed - you deserve a medal, or at the very least your guide to be made a sticky! Thank you.

Thanks to both of you for the thanks.
it should be possibly to get your commit charge below 150MB after a fresh boot before loading anything else.
cookies aren't bad, apart from tracking cookies (harmless but undesirable).
you can set your browser to block them by disabling third party cookies. - IE, tools, internet options, privacy, advanced, block third party cookies. firefox, tools, options, privacy, custom settings, untick accept 3rd party cookies, then you can also ok, and tick never remember history.
cleaning them with ccleaner every hour isn't really necessary, you can set ccleaner to run at boot, that's usually enough to keep a clean machine.
some remnants:
Install and run startuplite, accept suggested changes - http://www.malwarebytes.org/StartUpLite.exe
Disable ctfmon - control panel, regional and language options, languages, details, advanced, tick the Turn off advanced text services, ok
Click the java icon in control panel, updates, untick check for updates
unless you use yahoo toolbar, uninstall it.
Download and install cleanmem http://www.pcwintech.com/cleanmem (download direct download). In windows explorer, go to c:\windows\tasks, click on the clean system memory task, schedule, advanced, and change it from every 30 minutes to every 5 minutes, then ok, ok. Find c:\program files\cleanmem\mini_monitor, run it and right click the icon (near the clock) to set it to automatically run at startup, show percentage to keep an eye on your ram use. If your machine is still slow after doing everything listed, post your commit charge and installed physical ram details from task manager performance

Unless you need them running all the time, use the startup tab in msconfig to disable these items from running at startup (they can always be run manually if needed)

firefox is quite greedy on ram compared to ie, especially if you use plugins, it also check for updates by default which slows launching - tools, options, advanced, update, untick the 3 check for updates
In the firefox address bar, type about:config then right click, new boolean, create a key called config.trim_on_minimize set to true

This is a list of (or remnants of) security software evident in your log, you may wish to uninstall these in Control Panel (add/remove programs or programs/features) to keep your PC running smoothly, too much overlapping or bloated security or useless tuning software can have a drastic effect on performance - always leave one resident scanner running (eg avast/avira etc)
