debt management making debts sooooo confusing, help!!

Hello My husband went to a debt management company a few years ago when we seperated, now we are back together and im in control of the finances he cant cope with finances due to mental heath issues.
Anyway....last year i called them told them he was leaving, wrote to the creditors told them i was now going to pay them direct. All was fine until i started looking at the debts. It seems they invented a nat west debt from know where! that has now been sorted.
now have what appears to be one debt, but two companies collecting money for it. It gets very confusing as they all have their own refernce numbers. I have a letter from them dated 09, saying a debt was being passed from "Company A" to "compnay B", Company b is now being paid, suddenly 6 months later they decide to pay "company A" aswell. ggrrr Company A denys ever passing the debt to anyone.
Also on statement it show them sending for exapmle £19.99 but the creditor receives £19.95.?? only pennies i know but it doesnt instile confidence.
They now do things like send me the first page of a letter, to time waste i assume. They told my husband not to negociate debts, let them do it for a large fee of course!
Has anyone else had this problem, im not sure how to get to the bottom of it.

Hi
I would send a subject access request to kensington under the data protection act. It costs £10 but they should then send husband a copy of everything they have on file for him, all letters, all statements etc etc.
