Statement of Earnings

Hi,
My girlfriend has been trying to sort out the reclaiming of tax as she's been on the Basic Rate tax code, now HMRC has all the info from P45/P60s from various different companies through the year (temp work/agency before current work).
However the one piece of information they do not have is JSA.
To our recollection she signed on but was told that she was not entitled to anything and so was ignored, I believe the P45 was taken to Job Agency so that is no use to us.
And thus we are told to get a Statement of Earnings from there, how exactly would we go about doing so? Is this something that can be got from the local branch or do you have to call/write to a central office? I can't seem to find anything about how to do it online, simply that you may have to!
Any help would be greatly appreciated!
