Removing an ex partner from mortgage

Hi All a work college of mine wishes to have her ex partner of some 4 years removed from the mortgage and relinquish any claim to the equity in the property (small as it is), she has agreed a settlement figure with her ex, but can the mortgage be transfered to her in sole name?

If she can afford the payments on her own, she could consider a transfer of equity.0
