All you need to do is dress conservative. Women should have their whole body covered including legs and arms and it is preferred that they wear a scarf on their heads. Special clothing is not required. Loose long bottoms and loose long sleeved shirt buttoned up is fine. Face does not need to be covered. Regarding your cap it doesnt matter either way.
There are various titles that can be used for the Imam (prayer leader) and it also depends on which ethnic group the mosque belongs to. If you must use something arabic/islamic then you will be fine with calling him 'Shaikh' regardless of the ethnic group. Alternatively you call just call him Mr. XXXX, sir, mate, son, uncle etc.
Generally muslims are not stuck up on formalities and stuff too much so dont worry about it.
