Lost Savings AC passbook

I have £5.00 in a Nationwide cash ISA account but I've mislaid the passbook. I want to withdraw it. Previously I could have transferred online to my Nationwide current ac and got to it that way, but as I'm on a DMP and Nationwide is one of my creditors with a £500 o/d as soon as I transfer into that they'll grab it, plus I destroyed my bank card at their request. How can I get this out?

Second and final LBM 01/01/11 Nearly got there but fell of wagon. HAVE to do it this time :mad:
