Well, firstly I agree with the answer above about the books. You simply can't afford it, so... sorry, but if it's not in the library, read what is!
Regarding your "over limit" fees.
(1) If this is your bank charging them, then you could start going down the route of reclaiming them (although you might want to wait until you are debt free to do this.
(2) Above all else, avoid these further. Illegal in many cases though the charges are, your bank can charge you a reasonable amount, and this is just money thrown away.
My technique was always to pay debts as soon as you get paid. You say your DH (what's DH by the way? Seen OH, never DH?) is self employed so this may be harder, but if you have a regular monthly salary into your bank account then contact all the companies you have a DD with and arrange for them to come out a day or so after. If you know you have other bills coming, then always make sure you have enough money left.
Another technique which worked for me was to physically take money out the bank for things like shopping. I allowed myself £30 a week for shopping, took it out in cash every Saturday and then put it in an envelope. You should always do whatever you can to move debt down the interest rate tree, irrespective of the amount owing on that card. So transfer all of the balance of the 18% card to the new 0% card, plus £40 of one of the 12% cards (be careful of any charges there might be for doing balance transfers.... this advice assumes it's free).
From here, you need to work out your "debt reduction" budget. Based on above, making all the savings you could, it appears you could be paying off up to £1000 a month - a massive amount. Then it's quite simple. In order, pay the minimum amount off everything other than the highest interest card. Once you've paid each one off move down the tree.
From my very quick calculation, if you can even afford an extra £300 a month, you will have halved your debt in a year, and be debt free by Nov 08.Says James, in my opinion, there's nothing in this world

ref this. You can order in any book from the library. Even if your county don't have it anywhere in their libraries, they can order a copy for loan, from the British Library.

Because your bank will be very p****ed off with you and probably ask you to leave.
