Credit report - advice needed!

Hi guys,
Hope this is the right place to post this. I have just come to the end of a long debt management plan, feeling pleased that I had managed to pay back my debts. I got a copy of my credit report from callcreditcheck, and there's several things I'm trying to check out as a result.
I have a financial link on there to my ex-husband, who is terrible at paying his bills but I'm not sure if we closed our old joint HSBC bank account. Any idea how I would be able to close it without any of the bank account details. I do know where my ex-husband is, although we now live 200 miles from each other.
Also I have two accounts on there that I have no recollection of, they are old accounts, a Lloyds TSB credit card and an unsecured loan from prudential. Both have very small balances showing (£41 & £8) but I want to get the accounts closed. I don't have full account numbers, only the last four digits of each from my credit check.

Does the HSBC account show on your credit report? if it doesn't then I would just file a notice of disassociation - you need to do this with all 3 credit agencies, experian, euqifax and call credit.
Regarding the other accounts - are they shown as defaulted? what was the date of default in each case?

Yes the HSBC account is showing on my credit check. I thought it had been closed, but apparently not. It doesn't appear to be in use, but I want to get it closed asap.
The Lloyds TSB credit card shows that it defaulted in September 2006. The prudential loan is not showing as having defaulted. I have contacted Payplan who were running my DMP, but they say they have no record of these debts, but according to my credit check they have been receiving regular payment. Unfortunately due to having to leave my previous home asap I have ended up with none of the paperwork.
I think this may be the first credit check I have ever done. I know now that I should have been doing regular checks, so that's another learning curve for me!0
