Census - Disclosure Scotland form

Hi - I've had a conditional offer for a Census Collector job, but I'm having problems filling in the Disclosure Scotland form. The first two pages are fine, but then it won't let me put in my maiden name in the "Other Names" section on page 3, so I can't complete the form. I have told someone about it over the phone, but they seem to be treating it as if it's a problem on my computer, whereas it looks to me as if it's a problem with the way the form is set up.

So am I the only one having this problem? It wouldn't be an issue for most men, because if you haven't changed your name you don't need to fill anything in; but it would be for most married women.
