Asking a creditor to cease contact by telephone...?

Hi all.
I am looking for a letter template to ask a creditor (Lloyds TSB) to stop all contact by telephone because I am fed up with them calling me everyday as I've entered a DMP and there is nothing more I can tell them!
I have managed to find it on here before, but I have been searching this morning without any luck. Apologies as this is probably a much-asked question, but I would be very grateful if someone could post a link to it.
