Old old debt

Hi i need some help i have received a letter saying i owe money from an old mobile phone debt from over 7 years ago, i seem to remember somewhere on here there was a template letter to send off that clears the chasing of this debt, i thought i saved a copy somewhere but it seems i have lost it, its just i am worried as i am now paralysed and in a wheelchair living in disabled accomodation with carers and struggle to cope financially on the benefits i receive..i realise the debt was my fault years ago but back then i was working, had a house and was fully fit and healthy, please please help, thanks
