stopping interest and charges

Hi
i am currently managing my debt through CCCS with a DMP
however Natwest are still applying interest and charges to an overdrawn account.
last year i wrote to them using a template (i thought it was from this site) asking them to stop charges and interest on another account which they did.
my problem is i cannot find the template.
it wasn't the reclaim bank charges letter.
it had some specific references to the banking code and the fact i had followed their guidelines in setting up the DMP
can anyone help
